The Social Worker in the Health Care Center (HCC) provides social services to residents, their families and significant others to enable them to deal with the impact of illness and extended care placements with a primary goal of enhancing self-direction and independence at the highest possible level of functioning. 

 Essentials Duties and Responsibilities:

  • Identify the medically related social and emotional needs of all residents.  Make referrals for residents to social, health and community agencies as appropriate, following up with documentation of results of the referrals.
  • Complete admission paperwork with residents and/or families; review HCC Handbook with them.  Review residents' rights with residents and their representatives/ responsible parties. 
  • Maintain contact with family (with resident's permission) to report on changes in health, current goals, discharge planning, and encouragement to participate in care planning.
  • Help residents obtain supplies and clothing, whether through family referral, online or in-store shopping; assist in finding lost or missing items.
  • Participate in resident care management to include the following:  Initial and quarterly assessment using the MDS system; participation in regularly scheduled patient care plan conferences; assist in the development  of a multi-disciplinary plan of care; and review and revise, as necessary, the plan for social service intervention.
  • Provide alternatives to drug therapy or restraints by understanding and communicating to staff why residents act as they do, what they are attempting to communicate, and what needs the staff must meet.  Arrange for provision of needed counseling services.
  • Perform timely documentation of social service interventions and assessments in resident's chart.
  • Assist with discharge planning services as needed (for resident's return to independent living when possible).
  • Coordinate Resident of the Month, using resident's social history and family, working with Activities Director as appropriate.
  • Complete pre-screenings (Cornell & Geriatric Depression Scale, SLUMS, and Functional Assessment Staging) for new SCC residents; complete initial ISP; update as necessary.
  • Perform any other related duties as required or assigned.


  • Bachelor's degree and Arkansas license in Social Work plus 12 to 18 months related experience and/or training.
  • Ability to develop and maintain positive relationships with residents, families and staff.
  • Genuine care for and interest in elderly in a health care facility.
  • Must be compassionate and patience.
  • Excellent oral and written communication skills.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Ability to organize & prioritize job responsibilities.
  • Organized, professional, detail oriented, goal driven, and ability to multi-task.
  • Basic MS Office skills including Excel and PowerPoint.
